<p>Job: <strong>Home Ostomy Care Nurse</strong></p><p> </p><p><span>A home health nurse is responsible for </span><span>going to the patient’s home to administer their services and help patients maintain their independence</span><span>. </span>The ideal candidate will not only be skilled in assessing patients and providing care but will also use critical thinking and good judgment to integrate objective data with the subjective nature of each patient.</p><p><span> </span></p><p><span> </span></p><p> <strong><u>Requirements:</u></strong></p><ul><li>Bachelor’s degree in nursing with a license (RN)</li><li>At least 2 years of experience working in Clinics or Hospitals</li><li>Preferrable with hands-on experience in Ostomy and Wound care.</li></ul><p><br></p><p> <strong><u>Responsibilities:</u></strong></p><p><br></p><ul><li><span>Knowledge and treatment skills of acute and chronic wounds </span></li><li><span>Care for all open wounds – including changing, cleaning, and dressing.</span></li><li>Data collecting for patients.</li><li><span>Collaborates with the patient, family, and members of the health care team to develop an individualized plan of care. </span></li><li><span>Assesses patients’ and families’ learning needs and readiness to learn.
