As a Home Care Planning Professional within our regional health authority, you will be at the forefront of advancing homecare services and coordinating home care initiatives for the community.

Your primary focus will be on developing and implementing strategies to deliver high-quality healthcare remotely through telecommunication technologies, as well as optimizing home care services to support patients in their homes.

By leveraging technology and fostering a patient-centered approach, you will play a critical role in improving healthcare accessibility, enhancing patient outcomes, and promoting independent living for individuals in need of care.

Key Accountabilities & Activities:

  • Home Care Coordination: Coordinate and manage home care services for patients, ensuring a seamless transition from hospital to home and facilitating ongoing care needs.
  • Technology Integration: Collaborate with IT and telecommunication teams to ensure the integration and functionality of telehealth systems and home care technologies for effective service delivery.
  • Patient Assessment: Conduct comprehensive assessments of patients’ health conditions and care requirements to determine eligibility for telehealth services and home care support.
  • Care Planning: Develop personalized care plans for patients receiving telehealth services and home care, collaborating with multidisciplinary healthcare teams to ensure comprehensive and holistic care.
  • Education and Training: Provide training and support to patients, families, and healthcare professionals on the use of telehealth technologies and home care protocols.
  • Patient Monitoring: Utilize remote monitoring tools and data analytics to track patient progress, identify potential health issues, and intervene proactively to prevent complications.
  • Quality Assurance: Implement quality assurance measures to ensure that telehealth and home care services adhere to best practices, regulatory requirements, and quality standards.
  • Caregiver Support: Offer guidance and support to family caregivers, empowering them to participate in the care of their loved ones and promoting caregiver well-being.
  • Collaborative Partnerships: Collaborate with community organizations, home care agencies, and other stakeholders to strengthen the integration of telehealth and home care services in the overall healthcare delivery system.
  • Support in the management and delivery of sector projects, as required,
  • Support in the preparation reports for leadership on the current and forecasted progress/completion of key assets and initiatives, with executive levels insights on key assets, requirements, and risks
  • Plan and lead assigned tasks and projects, and proactively monitor and mitigate risks
  • Provide periodic reports regarding project progress, challenges, risks, and mitigation plans
  • Actively take part of and contribute in workshops and activities related to health system design activities
  • Support on administrative tasks as required for contract management
  • Assist in developing scope of work documents to support new contract establishment or renewal of existing contracts
  • Support in monitoring and evaluation of contract delivery and project performance
  • Support in all external communications activities (incl. internal/external marketing of the sector, maintaining promotional materials, developing strategic communications, etc.)

Background, Skills & Qualifications:

  • Proven experience in home care program development and home care coordination, preferably in a healthcare or regional health authority setting.
  • Knowledge of telehealth technologies, remote patient monitoring, and telecommunication systems.
  • Understanding of home care protocols, patient care planning, and geriatric care principles.
  • Excellent communication and interpersonal skills to engage with patients, caregivers, and healthcare teams effectively.
  • Proficiency in data analysis tools, electronic health record systems, and Microsoft Office Suite.
  • Ability to assess patient needs and develop individualized care plans.
  • Strong organizational and time management skills to manage multiple telehealth and home care initiatives.
  • Familiarity with healthcare regulations, privacy laws, and industry standards related to telehealth and home care services.
  • Bachelor’s degree in Nursing, Healthcare Management, Telemedicine, or a related field. A master’s degree is preferred.
  • Master’s in Healthcare system management, public health, business administration, medicine or related topics is beneficial
