<p><strong>DUTIES AND RESPONSIBILITIES</strong></p><p><br></p><ul><li>Provide complete, comprehensive, family-oriented outpatient care for patients throughout the life cycle, including health examinations and treatment of medical conditions, to include follow–‐up and any problems detected.</li><li>To manage acute, and chronic illness, develop a plan of care, coordinate and determine the need for a case conference.</li><li>Make appropriate referrals to internal disciplines in the center and external agencies for additional services required.</li><li>Work closely with the clinical team to provide continuity of care.</li><li>Responsible for accurate, timely documentation in the patient’s medical record of patient contacts, case planning and plan of care. All documentation should occur in the patient’s medical record.</li><li>Provide clear written and verbal communications to patients and staff, maintaining a culture of dignity and respect.</li><li>Participate in an on-call system if needed.</li><li>Participate in various administrative/clinical staff meetings to include, but are not limited to Continuous Quality Improvement Committee, general staff meetings, case conferences/management, and/or administrative meetings as requested.</li><li>Assist with planning to evaluate performance levels, quality service, productivity and future needs.</li><li>Communicate with care managers involved with each patient’s plan of care.</li><li>Occasionally participate in public relations functions on behalf of THB Home Health Care LLC.</li><li>Handles evaluation and treatment of all chronic diseases including pain management.</li><li>Handles patients of all age groups and deals with both acute and chronic diseases.</li><li>Examine patients for illness or injury, plan and provide treatment in the form of medicines and therapies. If necessary, refer to:</li></ul><ol><li>Specialists</li><li>Dietician</li><li>Physiotherapist</li></ol><ul><li>Provide education to patients and families regarding medical conditions and their treatment options as well as preventative care strategies of nutrition, exercise, and hygiene</li><li>Practice active listening and excellent communication skills with patients, their families, and all staff, addressing patient concerns as the highest priority</li><li>Document treatments on digital charting, adhering to THB’s medical charting standards, including ongoing progress notes and test results</li><li>Work with a multidisciplinary team (MDT) to collaborate on diagnoses and treatment plan</li><li>Order laboratory tests and interprets results</li><li>Coordinates with the THB homecare nurses on a daily call for medical updates of the patient’s condition</li><li>Coordinates with designated representatives and/or THB homecare patients on a weekly basis to monitor treatment and care progress.</li></ul><p> </p><p><strong>PROFESSIONAL RESPONSIBILITIES</strong>:</p><ul><li>Adheres to dress code policy, appearance is neat and clean.</li><li>Completes annual educational requirements.</li><li>Maintains regulatory requirements.</li><li>Reports to work on time and as scheduled, and completes work within the designated time.</li><li>Wears identification while on duty.</li><li>Manages and operates equipment safely and correctly.</li><li>Attends departmental meetings, and quality improvement as appropriate.</li><li>Complies with all organizational policies regarding ethical business practices.</li><li>Communicates the mission, ethics and goals of the facility as well as the focus statement of the department.</li></ul><p> </p><p><strong>PERSON SPECIFICATION</strong></p><p>Qualification:</p><ul><li>Medical Degree</li></ul><p>Experience:</p><ul><li>At least five (5) years of medical experience as a GP.</li><li>An interest in a home care setting is preferable.</li></ul><p><br></p><p><strong> Knowledge/Technical Skills:</strong></p><ul><li>Ability to understand and operate necessary computer equipment and software programs.</li><li>Two years Experience in Emergency Department or intensive care unit</li><li>Personal Abilities/Competencies:</li><li>Must be able to receive, interpret and follow verbal and written instructions. The ability to speak a foreign language is helpful, though not necessary. Must be self–‐motivated.</li><li>License/Registration: DHA License</li><li>Multilingual / English / Arabic</li><li>UAE Drivers License (Preferred)</li></ul>
