Job Summary
To be a primary source of scientifically valid information. To ensure the safe, appropriate, and cost-effective use of medications and advise physicians and clinical team on matters pertaining to drug usage and control within the hospital locations.
To participate in the development and delivery of continuous education programs for the pharmacy, patient care services, and medical staff and in maximizing therapeutic outcomes for patients. They will monitor antibiotic usage.
Job Responsibilities 1
Supports hospital policies and serves as a professional role model for all staff
Contributes to the development and execution of the Pharmacy department’s short to mid-term plans to ensure alignment with Allied Health’s strategic priorities and contributes to the development of annual business plans for the assigned areas of responsibility to support the achievement of financial objectives.
Provides input into the development and implementation of the pharmacy department’s policies, systems, processes, and procedures, identifying potential areas of improvement, to support an efficient and effective operation.
Participates in the development of new order sets and clinical practice guidelines for hospital-wide implementation.
Develops clinical pharmacy programs according to policies and regulations.
Collaborates with healthcare professionals to ensure optimal patient care.
Provides therapeutic recommendations and answers medication and patient-related questions from other healthcare providers, including providing drug information to prescribers and nurses regarding drug therapy and safe handling of medications.
Reviews records of patients to determine the appropriateness of medication therapy.
Performs chart review and evaluates patient’s condition to ensure all issues are being treated and advises the physician accordingly.
Assesses the results of pharmaceutical treatments.
Job Responsibilities 2
Responsible for evaluating and monitoring drug therapies and antibiotic utilization, developing treatment plans, initiating, titrating/tapering, and discontinuing medications under protocol, following up on necessary lab tests under protocol, and suggesting alternative medications in coordination with physicians.
Participates in formulary control and monitoring.
Performs Drug Information and Poisoning review and assessment.
Gathers and analyses data for selected drug utilization reviews.
Monitors, reports, and analyses adverse drug reactions and medication errors.
Identifies, resolves, and prevents potential and actual medication-related problems including untreated indications, improper drug selection, sub-therapeutic dosage, overdosage, failure to receive medication, adverse drug reactions, drug interactions, and medications without indication.
Consults on dosages, medication substances, correct administration of drugs etc.
Performs patient education functions on the appropriate use of medications.
Participates in the development and update of any required drug libraries.
Participates in the development of hospital-wide clinical protocols.
Ensures correct practice of handling controlled and narcotic medications according to the local laws and regulations.
Additional Responsibilities 3
Job Knowledge & Skills
Clinical pharmacy residency – Preferred
Clinical pharmacist licensure from MOPH in Qatar
Strong knowledge of protocols, policies, standards, and local laws and regulations.
Knowledge of actions, side effects, appropriate use of commonly employed drugs, antibiotic utilization, evaluation, drug therapeutic monitoring, adverse drug reaction monitoring, patient chart review and selection of alternatives.
Knowledge of current technology and pharmacy systems for distribution of medications
Knowledge of pharmaceutical and medical terminology abbreviations and symbols.
Job Experience
Minimum of 3 years of experience as a clinical pharmacist in a hospital setting
Hands-on experience in direct patient care
Minimum 2 years in GCC (preferred)
